This policy explains what this website collects, who processes it, and how long it is kept. It covers drjeromepeterson.com only — not treatment records, which are governed separately by state and federal medical privacy law.

What this website collects

The site sets no cookies and requires no account. The only personal information it collects is what you type into the contact form: your name, a phone number or email address so the office can reply, and the content of your message.

Please do not include detailed medical information in the contact form.It is an ordinary web form, not a secure patient portal. Clinical details belong in a phone call or an in-person visit.

Who processes it

Contact form submissions are transmitted to and stored by JotForm, a third-party form provider, which then delivers them to the practice. Your submission passes through and is retained on JotForm's systems, subject to their privacy policy.

The site is hosted by Cloudflare, which also provides its traffic statistics. Those statistics are aggregate only — page views and referring sites. They set no cookies, do not follow visitors between websites, and do not build a profile of you.

No other analytics, advertising, or tracking services run on this site. Personal information is not sold, and it is not shared for advertising.

How long it is kept

Enquiries are retained for as long as needed to respond and to keep a record of the referral, and are then deleted. If you would like your enquiry removed sooner, ask and it will be.

Links to other sites

Some pages link to outside organisations and professional registries. Those sites are operated by others and have their own privacy practices, which this policy does not cover.
